Medical facilities and health care providers who care for newborns and infants and are required to screen and evaluate newborns for CCHD and report data related to CCHD screening.

Reporting of positive screenings is required within seven days of obtaining a positive screening result through the online CCHD database at http://cchd.pqcnc.org or via fax to the NC Birth Defects Monitoring Program. The fax back form that should be used if positive screening results are sent to the NC Birth Defects Monitoring Program is available here. Aggregate data related to CCHD screening and evaluation must be collected and reported to the CCHD database at the end of each quarter. The public comment period on the proposed permanent rules will be held from December 2, 2014 through January 30, 2015.
