Hospital improvement teams participating in the Care of the Late Preterm Infant initiative met in Raleigh, NC, to kick off Year 2 and begin with a focus on improving the discharge planning process. Teams will continue to work on improving feeding support for this population both during hospitalization and after discharge. We have made great strides in the first year of the project in decreasing readmission rates for this population, and we will continue working to sustain those gains throughout 2025.
