1) Registration links for upcoming webinars will appear in the initiative weekly email and will be available online on your initiative page at pqcnc.org

2) Clicking the link will take you to the registration page - please fill out the page completely (using your own information) and click the register button.

3) If you have successfully registered you will receive an email confirming your registration along with useful instructions for accessing and participating in the webinar.

4) On the day and a few minutes BEFORE the time of the webinar click the link to join the webinar

5) Note that you have a CHOICE of audio options.

6) If your computer has a SPEAKER and a MICROPHONE the easiest option is to use your computers audio.  If you do not have a speaker and/or microphone on your computer you will need to call in using the information provided.

7) Once you’ve joined the webinar you will see two screens - a control panel and a viewer.

8) The viewer will display the content for the webinar and prior to the webinar starting will again provide the call-in number for the webinar if you are not using your computer audio as well as the audio pin for the webinar.  If you are using your phone you MUST enter the audio pin to participate.

9) From the control panel you can

18) When you have completed your presentation the organizer will take back presentation mode or passing it on to someone else and you will once again be a participant and no longer be sharing your screen

 

All team members listed on your roster should complete the Patient / Family Post Meeting Feedback survey once per month for the duration of the initiative. If there are other members participating as part of your PQIT, who are not listed on your roster, please update your roster and ask them to complete the survey as well.

Data Submission Due Dates: Please have all initiative data submitted by the 10th of every month for the previous month i.e. all January data should be submitted by February 10th. 

Attention Data Collectors:  Instructions on how to obtain access to Delphi to submit data.

Roster Changes: Please make sure to send me any changes in your team roster.  We would like to be able to provide all team members with the latest information about the initiative and we can only do that with your help.

Webinars: Monthly webinars will be held on the 2nd Tuesday of every month at 12:00pm beginning Tuesday February 11th.  Please go ahead and reserve these dates on your calendars.  Your team will need to schedule at least monthly meetings to review data, make plans for PDSA cycles, and plan for next steps.  Please set up times to meet with your Executive Champions to update them on your progress and to ask for their help to meet your goals. PQCNC would suggest you meet with them every other month but no less than every quarter.

A series of 8 online self-paced modules designed to help bring teams up to speed on basic principles of quality improvement work.  Topics covered include Assembling the Team, Patients and Families as Partners, Communicating the Work, Understanding the Problem, PDSAs, Data Collection, Understanding the Data, and Sustaining the Gains. 

Although initially these modules will only be available for teams who will be participating in the next round of initiatives, we anticipate (after feedback and further testing from our teams) making these modules available to any interested team. If you'd like to be updated when that happens use the 'Contact' link on the home page to let us know.
