Regional learning sessions were held in both Raleigh and in Winston Salem for registered teams from 26 hospitals across North Carolina who are committed to increasing the number of mother/baby dyads who exclusively breastfeed in the maternity stay. Teams worked to create plans to test activities believed to be potentially better practices when they returned home. Didactic sessions were led by Miriam Labbok, MD and Emily Taylor, MPH who are leading the initiative with Angela Pittman, RN, MSN. In each of the regional meetings, family faculty spoke with the teams about the experience of care. While testing new ways of delivering care, nurses are continuing to collect information about care practices and processes during the maternity stay. These teams are meeting monthly during the active phase of the initiative and sharing what’s working as well as what’s not working, offering each other support and reflection on activities and data collection.

The United States Breastfeeding Committee (USBC) published a new toolkit, “Implementing Practices That Improve Exclusive Breast Milk Feeding,” which focuses on improving adherence to evidence-based best practices, which is ultimately reflected in rates of exclusive breast milk feeding.
The new resource is intended for maternity facilities choosing The Joint Commission’s new perinatal care core measure set.

28 Hospital Teams across North Carolina have gathered baseline information and are entering the data into a secure web based data center. Each unit will send 3-6 participants to the first learning session the third week of January. During the workshop, teams will plan the activities expected to increase the numbers of infants exclusively breastfed in the maternity stay. The potential impact for North Carolina babies and families can be gleaned from AHRQ Publication No. 07-E007 where the authors of this review screened over 9,000 abstracts about breastfeeding and infant and maternal health outcomes.  They summarize the findings of the studies so that readers have one place to go to learn about the many risks of NOT breastfeeding, including:  [for breastfed children] acute otitis media, non-specific gastroenteritis, severe lower respiratory tract infections, atopic dermatitis, asthma (young children), obesity, type 1 and 2 diabetes, childhood leukemia, sudden infant death syndrome (SIDS), necrotizing enterocolitis, and [for mothers who breastfed] type 2 diabetes, breast, and ovarian cancer, and postpartum depression.

Teams are currently collecting information about their culture and habits which are known to support that are known to support the mother-baby dyad to achieve exclusive breastfeeding in the maternity center. Each of the participating teams is hard at work collecting this information for 100 babies in 2010 and will soon be entering the data into a web based data collection program.  Teams have also registered for the learning session to be held in Raleigh and repeated in Winston Salem.

Currently 28 hospitals are collaborating to increase the exclusive use of human milk for the term newborn. This month these teams have been collecting baseline data in order to understand the current practices and rates of exclusivity. The initiative will kick-off officially with a learning session for registered teams in either Raleigh on January 18 2011 or in Winston Salem on January 20 2011.

This work will dovetail with the NC Division of Public Health initiative, the N.C. Maternity Center Breastfeeding-Friendly designation, designed to recognize maternity centers that have taken steps to promote, protect, and support breastfeeding in their organizations.  The new NC program awards a star for every two steps achieved in the Ten Steps to Successful Breastfeeding as defined by the World Health Organization and Baby-Friendly, USA.
